How to obtain a retail food license

   4. Mobile Food License Application
      
      1. Blank Commissary Letter
      2. Mobile Food Unit Requirements
2. Submit Layout Plans, Equipment Schedule, Menu, Plan Review Questionnaire & Plan Review Application for our review & approval
3. Pay all appropriate fees
4. Contact all appropriate departments
5. Notify our office for a pre-opening inspection

Once you have gained approval from all the appropriate departments, & have a license, you may open.

NOTE: Make sure you follow the ISDH Food Code Rule and read the Food Establishment Reasons for Closure. Also, for facilities using (or planning to use) a roll-up door to open the facility to outside elements, please read and follow the guidelines set forth in the Garage Door Requirement Letter.
